How to use it

1. 1

   If any initial screen positive. Check all 4 initial screening questions (BMI < 20.5, weight loss, reduced intake, severe illness) — if all are negative: rescreen weekly.

2. 2

   Full score. Disease score + nutrition score + 1 if age ≥70.

3. 3

   Score ≥3. Dietitian referral + nutritional support within 24–48h.

When to use it

ESPEN endorsed hospital tool

NRS-2002 (Kondrup et al.) validated in clinical trials as predictor of benefit from nutritional support.

ICU nutritional risk

Combined with acute severity: ICU patients with NRS ≥3 benefit from early enteral feeding.

Gastrointestinal surgery

Pre-op NRS ≥3: ESPEN recommends 7–14 days nutritional support before elective GI surgery.
