Corrected Calcium

Albumin-adjusted serum calcium.

Corrected Calcium calculator

Result

2.20mmol/L

Normocalcaemia (2.20–2.65 mmol/L)

0.5 to 5 mmol/L

10 to 60 g/L

How to use it

  1. 1

Albumin-bound calcium is ~45%. Low albumin → falsely low total calcium. Correction estimates ionised fraction.

  2. 2

    Formula. Corrected Ca = measured Ca + 0.02 × (40 − albumin g/L). UK units.

  3. 3

Ion exchange. For albumin <25 or >45, or acid-base disturbances, measure ionised calcium (ABG) directly.

When to use it

Hypoalbuminaemia
ICU, liver disease, nephrotic syndrome — total calcium always needs albumin correction.
Hypercalcaemia work-up
Corrected Ca > 3.0: urgent treatment. 2.65–3.0: work up for PTH/malignancy.
Post-thyroidectomy
Monitor corrected Ca for hypoparathyroidism q6h for 24h post-op.
