How to use it

Fasting samples (minimum 8h fast). Serum glucose in mmol/L, insulin in µIU/mL.

HOMA-IR = (glucose × insulin) / 22.5. Threshold for IR varies by lab and ethnicity: often > 2.5 or > 3.0.

Limitations. Single time-point; affected by acute illness, corticosteroids, recent food.

- HOMA-IR > 2.5 in PCOS supports metformin use.
- MASLD/NASH risk
- HOMA-IR correlates with hepatic fat and fibrosis in metabolic liver disease.
