How to use it

  1. 1

Worst values in first 24h of ICU admission. Use worst (most abnormal) value for each physiological variable.

  2. 2

Acute Physiology Score (APS) = 12 variables. Each scored 0–4. GCS: use 15 − GCS.

  3. 3

ARF: doubles creatinine score. Creatinine ≥1.5 in ARF is double-weighted.

When to use it

ICU admission benchmark
Compare actual vs. predicted mortality for quality improvement.
Research stratification
APACHE II commonly used in RCTs to match baseline severity.
Not for individual prognosis
Population-derived — clinical judgment remains paramount.

A reference for clinicians and students, not medical advice. A score supports a decision; it does not make one. Check every result against the patient in front of you and your local protocol.
