How to use it

  1. 1

NPI = 0.2 × size (cm) + nodal stage + grade. From pathology report.

  2. 2

Prognostic groups. Good ≤3.4: lowest adjuvant therapy threshold. Poor >5.4: maximum systemic therapy.

  3. 3

Supplemented by molecular markers. ER/PR/HER2 status and Ki-67, Oncotype DX recurrence score further refine risk.

When to use it

Adjuvant systemic therapy decision
NPI used alongside Oncotype DX/Prosigna to guide chemotherapy decision in early breast cancer.
Patient counselling
Explains prognosis in terms of tumour biology.
MDT discussion
NPI provides objective clinical input alongside molecular profiling.

A reference for clinicians and students, not medical advice. A score supports a decision; it does not make one. Check every result against the patient in front of you and your local protocol.
