How to use it

  1. 1

GFR: use Cockcroft-Gault or 24h urine. EDTA-GFR or Cockcroft-Gault acceptable. Some centres cap at 125 mL/min.

  2. 2

AUC target from protocol. Most common: AUC 5 with paclitaxel (NSCLC, ovarian). AUC 6 may be used in fit patients.

  3. 3

Calvert formula. Dose (mg) = AUC × (GFR + 25).

When to use it

Carboplatin-containing regimens
NSCLC, ovarian, lung SCLC, testicular cancer, breast cancer.
Dose reduction in renal impairment
GFR drives dose — automatic reduction in CKD without separate adjustment table.
Protocol verification
Always cross-check calculated dose with oncology pharmacist and the specific protocol.

A reference for clinicians and students, not medical advice. A score supports a decision; it does not make one. Check every result against the patient in front of you and your local protocol.
