Very low risk — median survival ~8.8 yrs

Cytogenetics
Bone Marrow Blasts (%)
Haemoglobin (g/dL)
Platelets (×10⁹/L)
ANC (×10⁹/L)

How to use it

  1. 1

Requires cytogenetics, BM biopsy, and CBC. Blasts from BM aspirate + cytogenetics from karyotype or FISH.

  2. 2

IPSS-R replaces IPSS (2012). Better stratification, especially for low-risk disease.

  3. 3

Risk group guides treatment. Very low/Low: supportive care (EPO, lenalidomide). High/Very high: HMA + alloSCT.

When to use it

MDS diagnosis and treatment planning
IPSS-R is the standard risk tool for newly diagnosed MDS (WHO 2022).
Transplant decision
Intermediate/High: refer for alloSCT assessment if fit.
HMA therapy initiation
Azacitidine: High/Very high IPSS-R — improves OS vs. best supportive care.
