FIB-4 Index

4 inputsHepatology and GI

Non-invasive fibrosis score for MASLD/NASH.

FIB-4 Index calculator

Result

1.10

Low — F0–F1, advanced fibrosis excluded (NPV ~91%)

18 to 90 years

5 to 1000 U/L

5 to 1000 U/L

10 to 600 ×10⁹/L

How to use it

  1. 1

Standard LFTs + FBC. No fasting required. Consistent with other LFT samples.

  2. 2

Cutoffs vary by age. Age >65: use < 2.0 as low cutoff to reduce false-positives.

  3. 3

FIB-4 < 1.30. Very high NPV for advanced fibrosis — can avoid further testing in low-risk patients.

When to use it

MASLD (NAFLD) screening
EASL/AASLD recommend FIB-4 as first-line non-invasive test.
Chronic viral hepatitis
Validated in HCV (F2–F4 prediction) and HBV.
Follow-up after treatment
FIB-4 improvement correlates with regression of fibrosis (SVR in HCV, SGLT2i in NASH).
