How to use it

Only new or worsening features in last 10 days. SLEDAI-2K scores features present at time of assessment regardless of duration (continuous version).

Neuropsychiatric features weighted highest (8 pts). Seizure, psychosis, CVA, vasculitis — urgent escalation.
- 3
Target score 0–4. LLDAS (Lupus Low Disease Activity State): SLEDAI ≤4, no major organ activity, prednisolone ≤7.5 mg/day.
When to use it
- SLE clinic assessment
- Serial SLEDAI guides immunosuppression escalation and tapering.
- Flare monitoring
- Increase of ≥3 points = flare; increase of ≥12 = severe flare.
- Belimumab/anifrolumab eligibility
- SLEDAI ≥8 (belimumab) or serologically active disease.
