Febrile neutropenia risk — low vs.

MASCC Score calculator

Result

21/26

Low risk (≥21) — oral antibiotics, possible outpatient

Burden of febrile neutropenia

How to use it

  1. 1

    MASCC ≥ 21 = low risk. Complication rate ~6%, mortality ~1%. Oral ciprofloxacin + amoxicillin-clavulanate.

  2. 2

High risk: hospitalise immediately. IV pip-tazo or carbapenem (if prior ESBL/Pseudomonas); add antifungal if >7d neutropenia.

  3. 3

G-CSF not routinely indicated. MASCC does not incorporate G-CSF — use ASCO/EORTC guidelines for prophylaxis.

When to use it

Febrile neutropenia (ANC < 0.5 × 10⁹/L + temp ≥38.3°C once or ≥38°C × 1h)
Oncology emergency — MASCC within 1 hour of presentation.
Outpatient treatment eligibility
MASCC ≥21 + no organ dysfunction + reliable patient: outpatient oral regimen.
Haematological malignancy
AML/SCT patients almost always high risk regardless of MASCC.
