How to use it

  1. 1

6 laboratory values from admission bloods. CRP ≥150 = 4 pts (single highest weight).

  2. 2

Score ≥6. Intermediate-high: CT/MRI to look for gas tracking along fascia planes.

  3. 3

Clinical context essential. LRINEC validated retrospectively. Do NOT use to rule out NF if clinical suspicion is high.

When to use it

Soft tissue infection with concern for NF
Rapidly spreading cellulitis, pain out of proportion, skin necrosis, gas crepitus.
Score ≥8
PPV 93% for NF — surgical emergency: debridement within hours.
Normal LRINEC does not rule out NF
Sensitivity ~80%. Clinical suspicion must always prevail.
