Gentamicin Q24h — Hartford nomogram

Check 6–14h post-dose level for HD dosing interval (Hartford nomogram chart).

20 to 200 kg

Drug

10 to 200 mL/min

How to use it

  1. 1

Use IBW if patient obese. Gentamicin: distribute in lean body mass. Use AdjBW = IBW + 0.4×(ABW−IBW) if very obese.

  2. 2

Hartford nomogram. Draw 6h and 14h post-dose levels. Point on nomogram chart determines safe interval (q24/q36/q48).

  3. 3

Traditional TDM. Pre-dose level < 1 mg/L (gentamicin), peak 8–10 mg/L; or OD dosing with Hartford nomogram.

When to use it

Serious gram-negative infections
HAP, VAP, febrile neutropenia, pyelonephritis: OD aminoglycoside provides concentration-dependent kill.
Synergy in endocarditis
Streptococcal/enterococcal IE: low-dose gentamicin 3mg/kg/day in 3 divided doses (NOT OD dosing).
Avoid in CKD < 30 mL/min without ID supervision
Ototoxicity and nephrotoxicity risk — specialist monitoring.
