How to use it
- 1
Baseline creatinine. Ideally stable value in last 7–365 days. If none, use lowest in-hospital value.

KDIGO criteria (any). ≥0.3 mg/dL rise within 48h OR ≥1.5× baseline within 7 days.
- 3
Staging by creatinine. 1×–1.5×: Stage 1. 2×: Stage 2. 3× or ≥4 mg/dL: Stage 3.
When to use it
- Inpatient AKI monitoring
- Stage 1: fluid optimise, stop NSAIDs/contrast/ACEi. Stage 2+: nephrology.
- ICU
- Stage 3 + oliguria >12h: consider RRT.
- Post-operative
- AKI after cardiac surgery correlates with 90-day mortality.
