Abstract
【Abstract (English)】Traditionally, antidepressants have been explained as ‘improving depressive symptoms by increasing the concentration of neurotransmitters such as serotonin and noradrenaline in the synaptic clefts of the brain’. However, the 2022 Umbrella Review (Moncrieff et al.) scientifically demonstrated that ‘there is no consistent evidence of a serotonin deficiency in the brains of people with depression’, thereby invalidating the conventional core mechanism (the serotonin hypothesis). According to the latest neuroscience research (Knudsen et al., 2023), the fundamental mechanism of action of antidepressants lies in ‘modifying receptor sensitivity (the forced induction of upregulation)’. However, ‘true depression’, which is characterised by receptor downregulation, naturally undergoes upregulation upon the removal of excessive stress; therefore, forced induction via medication actually disrupts receptor homeostasis and leads to secondary depressive relapse. Furthermore, the ‘activation of the descending pain inhibitory system (analgesic effect)’ exhibited by antidepressants provides evidence of their anxiolytic and central inhibitory properties mediated via GABA-B receptors; according to this theory, this induces excessive pruning of frontal lobe synapses. Consequently, administration to minors or patients with ‘genetic predispositions to stress (vulnerability associated with developmental disorders)’ exacerbates schizophrenia-like pathology and inevitably triggers an activation syndrome (suicidal ideation, agitation, aggression).
