Padua Prediction Score (Medical VTE) calculator

Result

0points

Low risk (<4) — mechanical prophylaxis; reassess daily

How to use it

  1. 1

11 weighted risk factors. Active cancer, prior VTE, immobility, thrombophilia: 3 pts each.

  2. 2

Padua ≥4 = high risk. VTE rate ~11% without prophylaxis vs. ~2.2% with LMWH.

  3. 3

Bleeding risk assessment also needed. High VTE risk + high bleeding risk: mechanical prophylaxis (IPC) preferred.

When to use it

Medical inpatient admission
NICE NG89: assess VTE risk (Padua or IMPROVE) on admission for all medical patients.
Reassessment if condition changes
New immobility, cancer diagnosis, or procedure: re-score and review prophylaxis.
Discharge planning
Extended prophylaxis (LMWH 14–28 days) for high-risk patients post-cancer surgery.
