Abstract
Background
Patients with hepatocellular carcinoma (HCC) that progresses after first-line transarterial chemoembolization (TACE) combined with molecular targeted agents (MTAs) and immune checkpoint inhibitors (ICIs) have limited therapeutic options. This single-center, retrospective, single-arm real-world study evaluated the efficacy and safety of salvage yttrium-90 transarterial radioembolization (Y-90 TARE) plus second-line systemic therapy in this population.
Methods
We retrospectively identified HCC patients receiving Y-90 TARE combined with second-line systemic therapy between July 2022 and December 2024. Tumor response was assessed per mRECIST. Objective response rate (ORR), disease control rate (DCR), progression-free survival (PFS), overall survival (OS), and treatment-related adverse events (TRAEs) were analyzed. Albumin-bilirubin (ALBI) scores were obtained at baseline and 1 month post-TARE to evaluate hepatotoxicity, with baseline scores used for prognostic stratification.
