Abstract
Glaalpha Combination Ophthalmic Solution is a glaucoma medication composed of two active ingredients: Ripasudil hydrochloride hydrate 0.4% (a ROCK inhibitor) and Brimonidine tartrate 0.1% (an Alpha-2 agonist). Glaalpha represents the first Fixed-Dose Combination (FDC) eye drop to combine a ROCK inhibitor with an Alpha-2 agonist, thereby achieving three distinct mechanisms of action for lowering intraocular pressure (IOP) within a single bottle. These include the mechanism of Ripasudil, which enhances aqueous humor outflow through the conventional outflow pathway via the trabecular meshwork and Schlemm's canal, combined with the mechanisms of Brimonidine, which inhibits aqueous humor production at the ciliary epithelium while simultaneously enhancing uveoscleral outflow. The integration of these three mechanisms results in Glaalpha achieving an IOP-lowering efficacy from baseline of approximately 25%–35%, which is higher than either of its two constituent monotherapies. Specifically, Glanatec (ripasudil monotherapy) demonstrates an efficacy of approximately 20%–25%, while Brimodin (brimonidine monotherapy) demonstrates an efficacy of approximately 18%–25%. This difference reflects the synergistic effect achieved by acting through multiple mechanisms simultaneously within a fixed combination, making Glaalpha a highly effective agent for comprehensive intraocular pressure control.
