Abstract
Background
Benign prostatic hyperplasia (BPH) is a common condition among ageing men and a leading cause of lower urinary tract symptoms (LUTS) in sub-Saharan Africa, where delayed diagnosis remains a major challenge. In Benin, published data are scarce, particularly from La Croix Hospital of Zinvié. This study aimed to characterise the clinical profile, management patterns, and prognostic factors of therapeutic failure in BPH based on a retrospective cohort of 588 patients collected between 2021 and 2025.
Methods
This was a retrospective, descriptive, and analytical study conducted from January 2021 to December 2025 at La Croix Hospital of Zinvié, including patients with LUTS and an ultrasound-measured prostate volume ≥ 25 g and/or abnormal PSA after exclusion of prostate cancer. Data were entered using Microsoft Excel 2019 and analysed with Epi-Info 7.2.5.0. Qualitative variables were compared using chi-square or Fisher’s exact tests, quantitative variables using t-test or Mann-Whitney U test. Clinically relevant variables were entered simultaneously into a backward stepwise multivariate logistic regression model to identify independent predictors of therapeutic failure, a composite outcome defined as secondary surgery after failed medical treatment or a major complication within 90 days, with a significance threshold set at p < 0.05.
