Prevalence of high anti-A and/or anti-B antibody titres in donations collected from blood donors at the Regional Blood Center in Poznań
By Katarzyna Kolasińska.
Abstract
Introduction
High-titre anti-A and anti-B isoagglutinins represent an important challenge in low-titre group O whole blood (LTOWB) programmes. Data regarding donor-level prevalence and longitudinal variability of elevated isoagglutinin titres in Central European donor populations remain limited. Material and methods: A retrospective donor-level analysis was performed in 32,693 whole blood donors screened in 2019 at the Regional Blood Centre in Poznan. High-titre (HT) anti-A and/or anti-B antibodies were defined using an automated PK7300 screening strategy corresponding to a manual tube titre threshold of ≥1:128. Associations between HT positivity and donor characteristics were evaluated. Longitudinal analysis was performed in donors with repeated determinations.
Results
Overall donor-level HT prevalence was 7.10% (2,321/32,693). HT prevalence was highest among group O donors (13.76%), followed by group A (4.83%) and group B donors (2.46%), whereas no HT-positive donor was identified among group AB donors. Female donors demonstrated higher HT prevalence than male donors (14.66% vs. 6.81%; p <0.001). Among 16,943 donors with repeated determinations, 5.09% demonstrated variable HT status over time.
