Adolescent Reproductive Medicine and Personalized Gynecology: Clinical Pathway AGR-UA 2026 — Patient Route and Decision Gates (Ukrainian Adaptation)
By Viktoriya Hudziak.
Abstract
Adolescent reproductive medicine and gynecology span emergencies (ovarian torsion, ectopic pregnancy, heavy bleeding, sexual violence), pubertal and endocrine disorders, menstrual disorders, polycystic ovary syndrome (PCOS), premature ovarian insufficiency (POI), contraception and prevention, congenital anomalies, genetics, and fertility preservation. This pathway integrates them into one patient route with six decision gates, from emergency triage to transition into adult care. It includes a layer addressing adolescents' barriers to seeing a gynecologist and a "personalization" layer defined as risk stratification by measurable variables, not as a proprietary system. It is adapted to Ukrainian realities (primary-care gatekeeping, e-referral, wartime disruption, the 2026 immunization calendar with HPV vaccination). Recommendations are graded [A]/[B]/[C]/[EC] and rely on widely recognized guidelines, which must be re-verified against current editions before implementation. The construct of "AI-induced infertility" is not built into the pathway because causal evidence is lacking. This document does not replace clinical judgment or current Ukrainian regulatory acts.
