Abstract
Background
The treatment and surgical management of incisional ventral hernias remain among the most challenging and unresolved problems in abdominal surgery.
Objective
To conduct a scientific and analytical assessment of the efficacy of conventional autoplastic and modern alloplastic hernioplasty techniques used in the surgical treatment of incisional ventral hernias, with particular emphasis on the biocompatibility of prosthetic mesh materials (polypropylene and polytetrafluoroethylene), as well as the advantages and complications associated with different mesh placement techniques (onlay, sublay, and inlay). Results and Conclusion: In the management of large and complex abdominal wall defects, autoplastic repair using local tissues is associated with high recurrence rates (10–50%) and significant hemodynamic and respiratory complications. Synthetic monofilament polypropylene meshes, particularly when placed in the subaponeurotic (sublay) position, facilitate tension-free repair and substantially reduce the risk of hernia recurrence and wound complications.
