This was a second data analysis using publicly available resources. Library addresses were obtained at the Baltimore County public library website. Overdose data was obtained from Baltimore County Overdose Response website and was categorized by Zip code. Descriptive analysis was used to compare overdose deaths across neighborhoods. Results and Discussion: Naloxone was offered at all 19 libraries evaluated. As shown in Table 1, the number of overdose deaths per library ZIP code varied greatly. Between September 2025 to August 2026, North point and Sollers Point libraries (ZIP code 21222) in the Dundalk neighborhood (East Baltimore) were associated with highest numbers of overdose deaths (22 deaths). This was followed by Parkville (ZIP code 21234) with 15 deaths, Arbutus (ZIP code 21227), Essex (ZIP code, 21221) and Lansdowne (ZIP code, 21227) with 13 deaths each. Nine (47%) library served neighborhoods had no overdose deaths reported in the same period. The percentage change in overdose deaths from 2024 to 2025 also varied greatly among the neighborhoods served by these libraries. Ten library served neighborhoods showed a negative percentage change in overdose deaths. Only four neighborhoods registered a positive percentage change. Overall, there were 1,264 total overdose death across all the countries between September 2025 to August 2026, and approximately 20% decline in overdose deaths between 2024 and 2025. While Naloxone is equally distributed to all the county libraries, the volume of opioid cases varies across the various neighborhoods surrounding these libraries. This raises the question whether those heavily impacted regions (hot spots) should be receiving the same amount of Naloxone (same resources) or these hotspots should be given more resources in alignment with volume and density of overdose deaths. In this study, we call for an equity-focused distribution model supplemented with mobile dispensing units to cover those areas where overdose hotspots are dynamic and non-static. Future studies should focus on analyzing the utilization patterns of library-based naloxone among different demographic groups. Studies should also be focused on the overall death reduction attributed to this library-based distribution model and the overall community impact. References: 1. Jurczak, M. O., Dunn, K.
