Abstract
Preventive health programs in rural communities often occur as isolated events: residents are screened, receive brief advice, and the intervention ends. This community service article describes the implementation of a Screen -to-Action pathway linking free health screening with family nutrition and stunting-prevention counseling in Cikedokan Village, Bayongbong Subdistrict, Garut Regency. The program was conducted on 11 and 13 August 2026 through collaboration among Universitas Garut, village authorities, health personnel, Bakrie Amanah Foundation, and residents. The target group consisted of village residents, especially families, parents, pregnant women, breastfeeding mothers, and families with infants or toddlers. The available local data include location, schedule, partners, target group, materials, documentation, and process outputs, while participant counts, the number of toddlers or target families, clinical screening findings, and village-level stunting data were not recorded in the source document. Therefore, the findings are limited to service implementation, access to preventive information, and a follow-up design, rather than verified clinical change or stunting reduction. The model is replicable if future implementation adds attendance records, screening types, pre-post tests, referral logs, and family follow-up
