Abstract
Primary human papillomavirus (HPV) screening requires triage strategies that avoid overloading colposcopy services. The study therefore aimed to evaluate the efficacy of follow-up algorithms based on HPV genotyping to optimise colposcopy referral. A systematic review guided by PRISMA was conducted in PubMed/MEDLINE, the Cochrane Library and EMBASE from 2019 to 2025, with a sample of 15 publications. HPV-16 and HPV-18 genotyping identifies women whose immediate risk of grade 3 or higher neoplasia exceeds the colposcopy threshold, extended genotyping distinguishes intermediate-risk types, and p16/Ki-67 dual staining and methylation increase specificity. It was concluded that molecular triage optimises referral.
