Abstract: This study aimed to investigate the psychometric properties of the Human Papillomavirus Testing Knowledge Scale (HPV-TKS) among Turkish women.

Data were collected through an online survey conducted in Türkiye between September 2023 and February 2024, using non-probability convenience and snowball sampling.
A total of 234 women were initially recruited, of whom 205 met the inclusion criteria and were included in the final analysis. Eligible participants were women aged 30–65 years who were able to read and understand Turkish and provided informed consent. Women with a history of total hysterectomy or cervical cancer were excluded. Primary and secondary outcome measures: The primary outcomes were the validity and reliability of the HPV-TKS. Content validity was assessed using the Content Validity Index (CVI), and structural validity was evaluated through exploratory factor analysis (EFA) and confirmatory factor analysis (CFA). Convergent validity was assessed using average variance extracted, known-groups and discriminant validity were assessed using independent-samples t-tests and criterion validity was assessed using logistic regression. Reliability was examined using Cronbach’s alpha, McDonald’s omega and item-total correlations, along with floor/ceiling effects.
The mean HPV-TKS score was 2.67±2.10. CVI was found to be 0.94, indicating excellent content validity. EFA initially suggested a two-factor structure explaining 50.8% of the total variance (Kaiser-Meyer-Olkin test=0.807; Bartlett’s test: χ²=330.527, p<0.001). Using the same sample, CFA with the weighted least squares mean and variance adjusted estimator was subsequently applied to formally compare the one-factor and two-factor structures; the one-factor structure was retained based on its excellent absolute fit (χ²(20)=25.3, p=0.190; comparative fit index (CFI)=0.99; Tucker-Lewis index (TLI)=0.99; root mean square error of approximation (RMSEA)=0.036; standardised root mean square residual (SRMR)=0.066), consistency with the original scale and the absence of a conceptually meaningful distinction between factors, with item factor loadings ranging from 0.36 to 0.92. Internal consistency was acceptable (Cronbach’s alpha=0.76, McDonald’s omega=0.90). Item-total correlation coefficients ranged from 0.159 to 0.588.
The Turkish version of the HPV-TKS is a valid and reliable instrument for assessing women’s knowledge of HPV testing. The scale may support the identification of knowledge gaps and inform the development of targeted interventions aimed at improving cervical cancer screening uptake. Further studies are recommended to evaluate its applicability in diverse populations and its predictive value for screening behaviours.
