A narrative, evidence-informed synthesis was undertaken using a structured search of CINAHL, MEDLINE via PubMed, Scopus and Web of Science, supplemented by Google Scholar and purposive identification of professional, regulatory and governance sources. The original search was conducted on 12 April 2026, and a structured evidence update was undertaken to 17 September 2026 using the same six evidence domains and eligibility criteria. The synthesis included practising nurses, nursing students and nursing interns where smartphone use occurred in clinical or clinical-placement contexts, together with carefully identified transferable evidence from interruption science and cognitive psychology. Evidence was classified as direct, indirect/transferable or normative/governance evidence and interpreted using a Socio-Technical Systems framework. No formal risk-of-bias appraisal was undertaken; instead, credibility, directness and applicability were assessed during interpretation.
The updated evidence base confirms that smartphones have legitimate clinical and educational functions while also identifying increasingly direct nursing evidence concerning problematic use and distraction. Newly incorporated 2026 evidence includes a systematic review of problematic mobile-phone use among nurses and primary studies examining smartphone distraction, clinical decision-making, medical-error tendency, job performance, professional conduct during clinical placement, mobile-phone addiction and related psychosocial factors. Across this literature, the strongest recurring finding is association rather than causation: greater problematic use or distraction is associated with poorer self-reported or scale-based professional outcomes, while student and intern studies identify simultaneous benefits for learning and risks involving distraction, confidentiality, professionalism and infection prevention. The evidence therefore supports proportionate governance that distinguishes legitimate clinical use from problematic personal use rather than blanket prohibition.
The 2026 evidence update strengthens the empirical basis for treating problematic smartphone use as a nursing practice, leadership and organisational governance issue, but it does not establish that smartphone use directly causes patient harm. Nursing organisations should distinguish purposeful clinical use from avoidable personal engagement, provide secure and accessible digital alternatives, strengthen digital professionalism, and apply proportionate, non-punitive governance. Future research should prioritise prospective and intervention studies using objective clinical and patient outcomes. Implications for nursing practice: Nurses should use smartphones purposefully and in accordance with local policy, privacy, infection-prevention and professional requirements, particularly during safety-critical tasks and direct patient interaction. Nursing leaders should support this practice through clear expectations, secure organisational technologies, education in digital professionalism and proportionate responses to problematic personal use. The updated 2026 evidence supports concern about distraction and problematic use but also reinforces the clinical and educational value of appropriate smartphone use; governance should therefore be risk-based rather than prohibitive.
